In these cases, why isn't an infection usually detected from cultures of blood or tissue?

Bacteria are the second most common cause of artificial implant failures in humans..?
I worked in an OR for 2 years, and I've seen several such issues-it's usually dealing with the patient not taking their required medications post-op. Antibiotic treatment usually lasts for approximately 1 to 2 months after the operation, and anti-rejection meds are almost always taken for life.
